Intelligent Regulation of Smokeless Tobacco: Currently an Oxymoron




A commentary on “the political, social, genetic, and neuroscientific challenges in continuing an abstinence-only approach to nicotine use” was recently published by University of Ottawa Faculty of Law Adjunct Professor David Sweanor and student Adam Houston. 

In their Ottawa Law Review article (here), Sweanor and Houston observe that the “health catastrophe” of smoking “results not from the nicotine that smokers seek, but from the inhalation of smoke in order to obtain it”  They note that “non-combustion tobacco products — medicinal nicotine, various types of smokeless tobacco products, and now vapour products …effectively deliver nicotine without the inhalation of smoke,” and they believe that “shifting the market to non-combustion alternatives not only has the potential to achieve a public health breakthrough of truly historic significance, but is also a rather simple idea.”

According to Sweanor and Houston, “tobacco control policies hostage to an abstinence-only agenda” is so destructive “that even major US governmental health bodies avoid telling the public of the huge differential in risk between smokeless tobacco and cigarettes.”  I have repeatedly described the how the CDC doesn’t acknowledge the minimal risks of smokeless tobacco (here and here).

My decades-long effort to educate American tobacco consumers has been met with relentless opposition from anti-tobacco forces.  Sweanor and Houston show that wrong-headed opposition by health professionals is not unique to tobacco harm reduction.

Vaccines have been challenged for over 200 years.  “Dr. Edward Jenner is rightly honoured for the simple idea that cowpox [vaccination] could protect against smallpox, but his breakthrough in understanding the importance of vaccinations faced opposition…from those who saw such things as interfering with the will of God… As a result, widespread immunization took many decades to achieve.”

Physicians rejected hand-washing.  “The ground-breaking work of Dr. Ignác Semmelweis, on the simple idea of doctors washing their hands between conducting autopsies and attending to childbirth, was not merely ignored for decades, but his sanitary procedures were actually abandoned on the orders of his superiors, who did so after his measures had proven exceedingly effective at preventing an epidemic of unnecessary maternal and child deaths.” (emphasis in original)

Sweanor and Houston call for “intelligent regulatory oversight,” but it’s an oxymoron in the U.S. today, as evidenced by recent FDA actions on smokeless tobacco.  The FDA has conducted a misinformation campaign on cancer risks (here) that includes the conflation of “tobacco” with “smoking” (here).  Now, according to a report in The Hill (here), the FDA wants to lower the level of one or more tobacco-specific nitrosamines (TSNAs) in smokeless products. 

As I documented six years ago (here), TSNAs “are present [in tobacco products] in vanishingly small concentrations – mainly in the single-digit parts-per-million range… Despite decades of epidemiologic research, long-term use of Swedish or American smokeless tobacco products (other than dry powdered snuff) has not been associated in a significant way with ANY disease.”

I predicted that “anti-tobacco extremists will call for reductions. However, since there is virtually no evidence that current TSNA levels are associated with ANY measurable cancer risks, it is inconceivable that a reduction in levels will produce any measurable benefit.”

The Hill reports that the Campaign for Tobacco-Free Kids is taking credit for the FDA proposal.  CTFK’s Denis Henigan said, “We wrote to the agency and said well look if these products have such lower levels than clearly it's feasible to manufacture products with lower levels of carcinogens and you should draft a product standard.  It appears FDA has proposed to do something like that, something we asked for or close to it.”     

It is wrong for CTFK to drive FDA action in the absence of any scientific rationale related to risk reduction, when current risks are near zero.

FDA and CDC Generate Smokeless Scaremongering



While the FDA and CDC refuse to acknowledge that smokeless tobacco causes almost no disease, agency staff have produced a fresh study (abstract here) showing that smokeless tobacco users have higher trace levels of nicotine and some contaminants than do smokers.  This meaningless finding (explained below) leads to grossly misleading headlines, such as “Smokeless Tobacco More Toxic Than Cigarettes, Study Says” from Time’s Alexandra Sifferlin. (Other recent tabloidish stories from this reporter include “Flavor Science Explains How You Can Hear the Way Your Food Tastes,” “I Tried It: A 6 am No-Booze Dance Party on a Boat,” and “Healthiest Halloween Candy”).   

The media was served up a juicy quote from the FDA’s Brian Rostron, the study’s first author, in a press release (here): “exposure to nicotine and the cancer-causing tobacco constituent NNK were higher among exclusive smokeless tobacco users than exclusive cigarette smokers. This continues to put smokeless tobacco users at risk for adverse health effects, including cancer.”

First, nicotine is not a toxin, and it doesn’t cause any “adverse health effects, including cancer.”  

Second, while the study found higher levels of NNK among smokeless users than smokers, that does not support the claim that “smokeless tobacco users [are] at risk for adverse health effects, including cancer.”  Decades of epidemiologic studies involving millions of Americans show that smokeless tobacco users’ risks are infinitesimal.  British researchers estimate that U.S. smokeless-related cancer deaths are zero.

Third, from 1999 to 2012, the period of the study, trace levels of NNK dropped by two-thirds in smokeless tobacco users.

And finally, the results show that smokeless tobacco users had lower levels of mercury (in their blood) and arsenic (in their urine) than nonusers of tobacco.  In fact, smokers also had lower levels of these metals.  The study authors did not discuss the health implications of these findings.       

Public health officials should focus on meaningful research and provide appropriate scientific context for their findings, rather than scaremonger via the media and deter smokers from transitioning to markedly less harmful smoke-free products.

Reality Bites the Dust in New Smokeless Tobacco Study



It’s surprising what passes for “science” in some journals today.  Chemical Research in Toxicology has published a study from the University of California’s Berkeley and San Francisco campuses making this extraordinary claim: “Children living with smokeless tobacco [ST] users may be exposed to carcinogenic tobacco-specific nitrosamines [TSNAs] via contact with contaminated dust and household surfaces.”

The research was an offshoot from a larger effort looking for an environmental cause of childhood leukemia.  Families who participated in the study were divided into three groups: non-users of tobacco, smokers and ST users.  The contents of dust bags from their household vacuum cleaners were analyzed for NNN and NNK, two TSNAs that are considered to be causes of cancer.

Almost every household – even those with no tobacco users – had NNN and NNK in their dust, but researchers found “higher” levels of these agents in every gram of dust from an ST home – about 5 to 10 nanograms.  Keep in mind that a nanogram is one billionth of a gram, and a gram is less than four one-hundredths of an ounce. 

Each gram of moist snuff contains about 5-10 micrograms of NNN/NNK, about 1,000 times higher than the dust in this study (see my previous blog entry).  At one can a day (32 grams), the typical ST user is exposed to 32,000 times more NNN and NNK than the hypothetical child who eats a gram of dust every day.  It is well established that long-term use of ST even at that level is not associated with a significant increase in risk for ANY cancer.      

The California exposure data may be accurate, but the findings are of no practical consequence, other than to serve as fodder for hyperbolic anti-tobacco claims.

Minnesota Researchers’ #EpicFail – Trying for a Smokeless Tobacco/Cancer Link



Publishing a study of tobacco-specific nitrosamines in American smokeless tobacco products (abstract here), Dorothy Hatsukami and colleagues at the University of Minnesota called for the FDA to issue “regulations…to reduce levels of…NNK and NNN in smokeless tobacco products to the lowest levels possible.”  The authors make illogical and unscientific claims in their article and media interviews (here).

Claim 1: “The majority of smokeless tobacco users in the United States are not aware of …the tremendous variability in the levels of these chemicals across brands sold in this country.”  Tremendous variability?  Dr. Hatsukami reported that, with the exception of two products, the level of NNK+NNN varied from 0.64 microgram per gram of moist snuff to 3.89 micrograms (or, if you prefer, parts per million).   

Claim 2: “The higher the level of exposure the greater the risk for cancer.”  This statement is meaningless, as exposure to these agents has not been linked directly to human cancer.  The studies cited in the article involve SMOKERS, who are exposed to thousands of other toxins.  This is like claiming that golfers have a high risk for concussion, based on data from the National Football League.

The authors fail in their effort to make a cancer connection with a rat study from 1986, conducted by Dr. Hatukami’s colleague and coauthor, Dr. Stephen Hecht (here).  He exposed the mouths of 10-week-old rats to enormous doses of NNN and NNK daily for over 131 weeks.  The experiment was so aggressive that 86% of the rats died by trial’s end.  Not surprisingly, Dr. Hecht produced more tumors in the rats getting high doses.  Another result was entirely unexpected: In rats given the same massive NNN and NNK doses in snuff extracts, tumor production virtually disappeared. 

It appears that smokeless tobacco contains beneficial agents that virtually cancel out the negative effects of even massive doses of NNN and NNK.  That is probably happening with dippers and chewers, even though they are only exposed to minuscule levels of nitrosamines. 

I recognized this important smokeless attribute a decade ago, when I published a research study showing that commercial tobacco products have moderate-to-high antioxidant properties, much like fruits and vegetables (discussed here).

There is one bright spot in the Minnesota study. The authors acknowledge “The lack of association between snus use in Scandinavian countries to [sic] oral cancer.”  If only they and the broader scientific community would acknowledge that this profile also applies to modern American smokeless tobacco products.
